Job Description

Overview:

The Case Manager is responsible for coordinating care and ensuring that clients receive the appropriate services and support needed to meet their health and wellness goals. This role involves assessing client needs, developing comprehensive care plans, and facilitating communication among service providers and clients.

Key Responsibilities:

  • Conduct initial assessments and evaluations to determine client needs and eligibility for services.
  • Develop, implement, and monitor individualized care plans in collaboration with clients and their support systems.
  • Coordinate services and resources, ensuring accessibility and continuity of care.
  • Maintain accurate and up-to-date client records in compliance with confidentiality and documentation standards.
  • Advocate for clients to access necessary services and support networks.
  • Conduct follow-ups and reassessments to evaluate progress and adjust care plans as needed.

Qualifications:

Strong organizational skills and the ability to manage multiple cases effectively. Excellent communication and interpersonal skills are crucial for successful collaboration with clients and service providers.

Requirements

Minimum Requirements:

  • Bachelor’s degree in social work, nursing, psychology, or a related field.
  • Minimum of 2 years of experience in case management or a related field.
  • Knowledge of community resources and services.
  • Strong organizational skills and the ability to handle multiple priorities.
  • Excellent written and verbal communication skills.
  • Ability to work independently and as part of a team.

Preferred Qualifications:

  • Master’s degree in a relevant field.
  • Relevant certifications in case management or related disciplines.
