Position Summary: Triage and partners with healthcare providers to ensure health and safety standards for out-of-home childcare, childcare licensing requirements, disease reporting requirements for care providers, immunizations for children, injury prevention for children, medication management and knowledge of community health and mental health resources for children.

- Coordinate team based medical care for the children placed in the center.
- Prepare, compile, submit, and maintain accurate medical records, files, forms, statistics, and additional relevant information in accord with agency policy, licensing and/or funding requirements.
- Review ORR portal for compliance with medical information upload requirements and ensure updates are input as needed for safety and timely discharge.
- Complete calls with sponsors as assigned to provide medical information.
- Actively participate in medical and service-related resource development prior to UC discharge.
- Participate in planning, coordinating, implementing, and evaluating all medical needs of the UC for designated social service program(s).
- Provide documented information related to identified delays in the medical assessments.
- Attend scheduled meetings and participate in team decisions and operations to represent the medical needs of the UC.
- Coordinate medical resources for all employees and resource family homes.
- Adhere to Child Abuse and reporting requirements.
- Must be relied upon regarding task completion and meeting deadlines without sacrificing accuracy, work quality, or customer service satisfaction.
- Attending meetings and training when needed or requested.
- Perform other duties as requested by the assigned supervisor and/or management.

- 2+ years of working in a healthcare facility setting
- 2+ years working experience with children and families preferred.
Education/Experience: Associate's degree or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
Associate degree preferred. Certified Medical Assistant (MA), Licensed Vocational Nurse(LVN) Registered Nurse (RN) , Bachelor of Science in Nursing license (BSN) and two to four years experience working with children and families in a healthcare setting. Language Ability: Bilingual in Spanish required. Read, analyze and interpret business, professional, technical or governmental documents. Write reports, business correspondence and procedure manuals. Effectively present information and respond to questions from managers, customers and the public.
Mathematical Ability: Add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Compute rate, ratio and percent, and draw and interpret bar graphs.
Reasoning Ability: Solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer Skills: - Proficiency in MS Office (Word, Excel, Outlook)
- Proficient in the use of standard office equipment.
Supervisory Responsibilities: Directly supervises employees in the Department. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.
Supervise, coaches, and mentors staff providing guidance to their work and learning including,
when necessary, corrective/disciplinary action in a timely and professional manner.
